Personal Development Plan

Your PDP is an important tool to begin your learning journey with us. It will ask you for information about yourself, your education history, and what you would like to achieve from the RUCN-CDP program. This PDP will be used by you and one of our RUCN-CDP Nurse Educators to ensure that you get the most out of the program and achieve your desired key learning outcomes.

The following steps are outlined to assist you in completing your PDP and progressing through to Module 1 of the RUCN-CDP.

Step 1 – Complete your PDP using the blue link below. This questionnaire should take you approximately 15 minutes to complete.

Step 2 – Save your PDP. At the end of the questionnaire, you will need to download your answers into a PDF file which will be saved to your computer in your downloads folder. Don’t worry, this is an easy process and the instructions on how to do this are at the end of the PDP questionnaire.

Step 3 – Upload your PDP below. You will need to upload the PDF file into the assignments section below.

1. Scroll to the bottom of this page and use the ‘upload assignment’ section.
2. Click the grey ‘browse’ button and find your PDP in your ‘downloads’ folder on this computer. The file name will start with ‘SurveyHero-Response’. 
3. Click ‘open’ at the bottom
4. This will take you back to this screen, then click ‘upload’. 
5. You will then see that your assignment is now ‘awaiting review’ and will be approved in your 1:1 meeting.

Step 4 – Book your 1:1 meeting. After you upload your PDP below, you can self-schedule your 1:1 meeting with any RUCN-CDP Nurse Educator using the calendar below. It is important to ensure you have uploaded your PDP below before this meeting as a RUCN-CDP Nurse Educator will go through your PDP and talk to you about the program. It’s a great informal way of meeting a support person from our team.
